 Clinical Record Cards
IACLE has available clinical record cards that may be of interest to its members. These record cards are based closely on the originals supplied by the School of Optometry, University of Waterloo, CANADA.
They are comprehensive and cover most clinical and administrative aspects of contact lens practice regardless of the setting. They are especially suitable for use in teaching clinics.

Because the circumstances of most IACLE members differ significantly from one another and from the University of Waterloo’s clinic, it is envisaged that these cards would not be deployed ‘as is’, rather they would be used as the basis of a pro forma record card customized to the needs of the individual institution. As inspiration for the development of such a customized card, these forms have much to offer.
